ENA's new Monday-Tuesday drama 'Dr. Brain' captivated viewers in its premiere episode, announcing the birth of a healing romance between Lee Jae-wook and Shin Eun-soo. The drama began with the life-altering news of 'Pyeondong Island assignment' for public health doctor Do Ji-han (played by Lee Jae-wook), depicting his chaotic adjustment to the island and his first encounter with Yuk Eun-ho (played by Shin Eun-soo). In particular, the scene where Yuk Eun-ho warmly embraces Do Ji-han, who is consumed by anxiety due to his trauma, heightened anticipation for the story to come.

The broadcast opened with a scene of reservist Do Ji-han skillfully treating a critically ill patient during basic military training. A former plastic surgeon at a university hospital, he was a 'public health doctor' but desperately wanted to avoid an island assignment. However, the unexpected news of his 'Pyeondong Island assignment' struck him.

Do Ji-han's arrival on Pyeondong Island was not smooth. While dependent on medication, he fell into the sea while trying to save Yuk Eun-ho, who was precariously standing on the deck. When he regained consciousness, he found himself at the Pyeondong public health center, a 'frail island boy,' and everything felt unfamiliar. Recalling Yuk Eun-ho's face, saddened by the discovery of targeted anticancer drugs in a swapped bag, Do Ji-han found himself concerned about her. However, Yuk Eun-ho revealed the shocking truth: contrary to Do Ji-han's memory, she was the one who carried him, intoxicated and hallucinating, to safety. Their unusual first impression hinted at a special connection.

Do Ji-han's hardships continued. The dilapidated medical equipment, the cockroach-infested dormitory, and the incident with his first patient, Park Chun-sik (played by Woo Hyun), made his island life even more difficult. Do Ji-han's judgment, suspecting myocardial infarction in Park Chun-sik's condition, which the patient insisted was simple indigestion, proved correct. Park Chun-sik collapsed but was saved through CPR and helicopter transport.

However, facing the situation of having to return to Pyeondong Island, Do Ji-han experienced extreme distress due to his trauma related to the sea. At that moment, Yuk Eun-ho approached him, put on headphones, and soothed his anxiety with music. Do Ji-han's recollection of his first meeting with Yuk Eun-ho, accompanied by the warning, "Three things to avoid on the island: people, incidents, and love," sparked a flutter of excitement. Anticipation grew for how the story of Do Ji-han, who fears the island due to past trauma, and Yuk Eun-ho, who unexpectedly appeared in his life, would unfold.
